Attachments must be original documents in PDF format, photocopied or scanned in colour, and high resolution to display all the information on the original document.
Scanning/photocopying must be perpendicular, as optical reading processes information. Furthermore, scanning/photocopying must cover all visible content on pages, including if something is written on the edge of the page.
All pages must accompany each document, even if only part of them is referenced.
Pages must be marked with a serial number. If original documents do not have page numbers, they must be labelled with a serial number and ensure they are in the correct order.
Copies of medical records must contain all forms, notes, research results, and entries, including handwritten notes, certificates, pictures, etc. All health records of the patient's discharge/treatment/surgery must be included. It shall be noted if something is left out and then why.
If it is impossible to comply with the above or if data is omitted, this should be stated with explanations.
Unclear/incomplete copies of documents will be rejected unless the sender justifies that better copies cannot be provided.
