How is an application for an assistive device in a car made?

In most cases, a healthcare practitioner must submit an application for an assistive device along with a checklist. If the person in question has a valid certificate from Iceland Health, anyone can apply.

For those who do not have access to the Data Portal (Gagnagátt), that is, other than healthcare professionals, you can fill out the form Applications and forms | Iceland Health - Assistive devices and sent in through Data submission of individuals or returned to our Service Center along with a checklist Checklists with Applications.
