Assisted Living Facilities for Older Adults

Before applying for placement at an assisted living facility for older adults the applicant must complete a needs assessment. The applicant's medical condition and personal circumstances are taken into consideration and an assessment made of their needs.

Needs Assessment for Assisted Living

Applications for placement are not made directly to assisted living facilities. Instead, an assessment must be completed to determine the need for placement.

The request is submitted to the needs and health assessment committee in the district where the applicant has their legal domicile. The committee has four weeks to complete their assessment.

The case is then reviewed to determine whether the applicant can continue in their own housing with increased support. If this is the determination, then the process comes to an end.

If it is determined that the applicant does not require placement at an assisted living facility, the decision is delivered along with the committee's reasoning for denying the application. The committee's decision can be appealed to the Ministry of Health.

If it is determined, however, that the applicant has demonstrated need for placement at an assisted living facility, the decision is delivered to the applicant and a copy forwarded to the applicant's assigned physician, in-home nursing staff and social services.

The applicant's needs assessment for assisted living is valid for twelve months. If the applicant has not been assigned a facility by the end of this period, a new assessment must be performed.

Requesting a Needs Assessment for Assisted Living

The administration and assessment teams for each of the municipalities' departments of services for older adults make a joint decision on placement in assisted living facilities, taking into consideration the results of the needs and health assessment committee.

Applications may be made for more than one assisted living facility anywhere in the country, with the applicant ranking their choices by preference.

When a place becomes available, the needs and health assessment committee provides the facility's administration with the names of two individuals, one of whom is offered the place.
